HealthPathways

HealthPathways is an online health information portal that supports local GPs and health professionals at the point of consultation. HealthPathways provides clinical decision support frameworks on how to assess and manage medical conditions, and how to appropriately refer patients to local services and specialists in the most efficient way.

The pathways are developed by local GPs, local hospital clinicians, and private allied health providers and clinicians. There are two programs in the CESPHN region – HealthPathways Sydney and South Eastern Sydney HealthPathways.

Sydney and South Eastern Sydney HealthPathways experienced strong clinician engagement during the 2025–26 financial year. The platforms provided GPs, allied health professionals and other primary care providers with locally relevant clinical guidance and information, supporting evidence-based care and informed referral decisions.
