Specialist geriatric outreach available for residential aged care homes across Sydney and South Eastern Sydney Local Health Districts.
During 2025-26, CESPHN-supported geriatric outreach services delivered more than 5,000 episodes of acute care across residential aged care homes in the Sydney and South Eastern Sydney local health districts. The service provided timely specialist assessment and clinical advice for older people experiencing acute deterioration, helping residents receive care in familiar surroundings where clinically appropriate.
The program strengthened collaboration between residential aged care homes, GPs and hospital-based geriatric teams, improving communication, continuity of care and support for aged care staff managing complex resident needs. These partnerships helped reduce avoidable emergency department presentations and hospital admissions while supporting better health outcomes and quality of life for residents.
”The After-Hours Geriatric Outreach Service helped our clinical team manage complex health issues with greater confidence and enabled more residents to receive the right care in the place they know and call home

Homelessness
CESPHN works with health, housing and community organisations to improve access to healthcare for people experiencing homelessness and housing insecurity. Through commissioned services, outreach programs and system partnerships, CESPHN supports integrated, person-centred care that addresses immediate health needs and the broader factors that influence health and wellbeing.
During 2025-26, CESPHN continued to support specialist homelessness health services delivering outreach, care coordination and multidisciplinary healthcare across central and eastern Sydney. More than 1,200 people accessed multidisciplinary health and care coordination through St Vincent’s Hospital Homeless Health Service, with more than 3,500 occasions of service delivered.
A further 700 people accessed health and care coordination through Kirketon Road Centre’s Inner City Outreach Service, with more than 2,500 occasions of service provided.
CESPHN also partnered with Sydney Local Health District and Newtown Neighbourhood Centre to better understand the health needs of boarding house residents. The project surveyed 81 residents from 30 boarding houses across Sydney’s inner west and provided care navigation in response to health, housing, mental health, income support, dental and aged care needs.
Nationally, CESPHN contributed to the PHN Homelessness Access Program, supporting collaboration across 31 primary health networks and contributing to national communities of practice and knowledge-sharing forums.
